Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced inspection was initiated and completed on 7/23/2025 in response to a complaint received by the licensing office on 7/7/2025 relating to allegations of supervision. The Department issued a Special Order on 1/17/2025 to require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations, and place the licensee on probation for a period of 6 months as a result of high-risk violations. No records were reviewed. This inspection included document review, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
