When staff are supervising children, they shall always ensure their care, protection, and guidance.
Staff did not ensure that Child 1 (age 4 years) who has a diagnosed food allergy, documented in the allergy care plan on file in the child's record, was protected from the allergen during snack on 1/30/2026. The child ingested the allergen which resulted in a reaction that required emergency medication to be administered onsite and additional outside medical treatment. The center noted that the allergy was "severe" in the incident report of the event.

Plan of Correction: The child #1 has been provided with a designated, closed plastic cup that is clearly identified as his and is only used by him. The child now sits in an assigned, consistent seat at the table during all meals and snacks to enable staff to better monitor food exposure. Staff directly supervising the child have reviewed his allergy care plan and emergency procedures.
