Plan of Correction: Maintenance will repair or replace the uneven playground tiles to eliminate the tripping hazard. The repair will ensure that all tiles are properly secured and level.

When children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
Staff 1 and Staff 2 had 18 children in Classroom 1 on 2/18/2026. The youngest child in the group was 2 years old. Three staff would be required.
