Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 1/15/2025, as a part of the licensure period. The on-site inspection began at 9:35am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. There were 81 children present and 18 staff. The inspector reviewed 8 children?s records and 5 staff records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually.
The information in the record of child #3 had not been updated in over 14 months, record for child #4 had not been updated for over 15 months, record for child #6 had not been updated in over 4 years.
Plan of Correction: Director and staff will ensure that children's records are reviewed annually with parents.

The center shall have a written emergency preparedness plan that is reviewed/updated annually.

The center's written emergency plan was last reviewed/updated 2022.
Plan of Correction: Emergency plan is in the process of being updated. Will be reviewed with staff upon completion on 2/17/25.

Shelter in place procedures shall be practiced a minimum of twice per year.

The center did not conduct shelter in place drills in the past year.
Plan of Correction: Director will ensure shelter in place drills are practiced at least twice a year.

Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that includes the time it took to complete the drill.

There was no documentation of the time it took to complete the evacuation or lockdown drills.

When food is brought from home, the food container shall be sealed and clearly dated.
None of the children's food containers or bottles in the Elephant 1 room were dated.
Plan of Correction: Labels for food brought from home will be updated to include date.
