Inspector Notes
An unannounced inspection was initiated on 2/26/2025 and completed on 3/10/2025 in response to a complaint report received by the licensing office on 2/24/2025 relating to allegations of administration, medication administration, and special care. The inspector reviewed 1 child?s record on-site. This inspection included document review, a tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, this complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, other violations were found related to administration, special care provisions and programs are documented on the violation notice issued to the program.

Violations
7and thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
Classrooms #2 and #3 did not have at least twelve inches of space between the sides and
ends of occupied cribs or thirty inches of space between service sides of occupied cribs and
other furniture.
Staff #5 did not wash their hands or the child?s hands after diaper change.
sanitized after each use. Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
Staff#5 did not clean or sanitize diapering surface after it was used and used their hands to dispose of a soiled diaper, instead of foot pedal.
Medication shall be kept in a locked place using a safe locking method that prevents access by children.
In Classroom #2, a child?s prescription medication was not stored locked.
Centers shall keep a record of medication given children, which shall Include the day and time the medication was administered to the child and any medication error.
Medication administration log indicated medication was given to child at 12:00pm, however it was not given at the documented time and error of time was not documented.
In Classroom #1, there were no protective belts on high chairs being used by 3 children.
