Inspector Notes

An unannounced monitoring inspection was initiated on and completed on 6/12/2025 in response to a self-report received by the licensing office on 5/22/2025 relating to supervision. The inspector reviewed 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 5/20/2025, Child #1 (2 years) was unsupervised for approximately 5 minutes on the playground. The child was found by staff #4 on the playground and returned to the school building with them.
Plan of Correction: Name to face retraining and reinforcement.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Child #1, who is 2 years old, was out of sight and sound supervision of their staff for approximately 5 minutes. Staff left the playground and walked around the building without this child.
Plan of Correction: The center has updated count sheet form to include exiting building, entering playground, exiting playground, re-entering building in addition to every 30 minutes.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by standards to identify where attending children are at all times.

Staff 1, 2 and 3 stated no name to face check was conducted when exiting playground.
Plan of Correction: The center has updated protocol regarding playground supervision, name to face count sheet/form and exiting/entering building.
