Inspector Notes
An unannounced monitoring inspection was conducted on 09/26/2022 from11:45 a.m. to 2:15 p.m. There were 52 children present, ranging in ages from 6 months to 5 years old, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
8Evidence:
1. Staff #3?s start date was 04/18/2022.
2. The date of completion for the Virginia Department of Education-sponsored orientation course for Staff #3 was 09/20/2022.
3. Staff #6 confirmed that Staff #3 did not complete the Virginia Department of Education-sponsored orientation course within 90 days of employment.
Evidence:
1. In Classroom #1, Equate shaving cream was observed in the bottom shelf of an unlocked storage closet. Staff #7 confirmed the shaving cream was not in a locked place.
2. In Classroom #2, commercial air freshener, water cleaning tablets and Mr. Clean floor cleaner were observed in an unlocked lower cabinet. Commercial air freshener and commercial disinfectant spray were observed in an upper unlocked cabinet. Staff #5 confirmed that the cabinets were not locked.
9/27/22. Teachers have been reminded that all chemicals must be locked, locks must remain locked and keys may not stay in locks.
Evidence:
1. In Classroom #2, 1 child was observed sleeping in a bouncer chair.
2. In Classroom #3, 2 children were observed sleeping in bouncer chairs.
3. Staff #6 provided the center?s safe sleep policy for review and confirmed that the center?s policy included that children should be placed for sleep ?in a supine position? and ?on a firm sleep surface?.
Evidence:
1. In Classroom #1, 11 containers of sunscreen were not labeled with a child?s name.
2. Staff #6 confirmed the sunscreens were not labeled with a child?s name.
Evidence:
1. Staff #5 was observed to be the only staff member in Classroom #2.
2. Staff #5 did not have CPR certification on file.
3. Staff #6 confirmed that Staff #5 did not have CPR certification and was the only staff member in Classroom #2.
