Inspector Notes
An unannounced focused monitoring inspection was conducted on 09/26/2023 from 9:30 a.m. to 10:00 a.m. There were 62 children present, ranging in ages from four months to four years, with 18 staff supervising. The inspector reviewed compliance in the areas of staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

REPEAT VIOLATION
Based on observation and interview, the center failed to ensure that children under the age of 10 years old are always within actual sight and sound supervision of staff.
Evidence:
1. Four children were observed in the upper part of the City Play Place while all four staff members were observed on the floor level of the playroom.
3. Staff #1 confirmed that the children in the upper part of the City Play Place could not be seen when from the floor level where the four staff members were.
4. Staff #1 confirmed that all the children in the group were two years old.
