Inspector Notes
An unannounced complaint inspection was initiated on 04/29/2024 at 3:05 p.m. and was concluded on 05/13/2024 in conjunction with local child protective services. The complaint was received on 04/29/2024 regarding staffing and supervision, and programs. Eight staff members were interviewed.
Information gathered during the inspection supports at least one of the allegations and the complaint was determined to be valid. One complaint related violation and one non-complaint violation were documented on the violation notice issued to the program.

Based on interviews, the staff failed to report any suspected abuse or neglect to the local Department of Social Services or the department?s toll-free child abuse and neglect hotline.
1. Staff #1 was observed smacking children?s hands on numerous occasions, sometimes resulting in the children crying. None of the three witnesses reported this to the local Department of Social Services or the department?s toll-free child abuse and neglect hotline.

Based on interviews, the center failed to ensure that behavior guidance is constructive in nature, age and stage appropriate and intended to redirect children to appropriate behavior and resolve conflicts.
Evidence:
1. Three staff have witnessed Staff #1 smacking the children?s hands as a form of discipline on numerous occasions. Two witnessed some of the children crying afterwards.
2. Five staff who had worked directly with Staff #1 had witnessed Staff #1 yelling at the children on numerous occasions. One staff member who did not work directly with Staff #1 heard, Staff #1 yelling at the children from another room.
