8VAC20-780 Physical Plant.

During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on November 21, 2024 and completed on November 21, 2024 in response to a complaint received by the licensing office on November 13, 2024 relating to allegations of physical plant. The Department issued a Special Order on August 22, 2024 to place the licensee on probation for a period of six months and to contact parents guardians and other responsible persons (families) regarding health and safety violations as a result of high-risk violations. This inspection included tour of the facility, interviews, and observation.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 5, 2024. A POC submitted after this date will not appear on the public website.

In inside areas occupied by children, the temperature shall be maintained no lower than 68 degrees Fahrenheit.
On November 21, 2024, the temperature in the Infant B room was 66 degrees Fahrenheit. The temperature in the Toddler room was 67 degrees Fahrenheit. The temperature in the Two?s room was 62 degrees Fahrenheit.
Portable heaters had been in use for a couple weeks and they were not sufficient in maintaining the rooms at 68 degrees Fahrenheit.
