Inspector Notes

An unannounced inspection was initiated on November 21, 2024 and completed on November 21, 2024 in response to a complaint received by the licensing office on November 13, 2024 relating to allegations of physical plant. The Department issued a Special Order on August 22, 2024 to place the licensee on probation for a period of six months and to contact parents guardians and other responsible persons (families) regarding health and safety violations as a result of high-risk violations. This inspection included interviews.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
