During the inspection, the inspector reviewed the areas listed . Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on January 29, 2025 and completed on February 12, 2025 in response to a complaint received by the licensing office on January 22, 2025 relating to allegations in the area of Staffing and Supervision, Programs, and Special Care Provisions and Emergencies. The Department issued a Special Order on August 22, 2024, to place the licensee on probation for a period of six (6) months and require the licensee to contact parents, guardians or other responsible persons (families) regarding health and safety violations as a result of high-risk violations. This inspection included interviews and observation.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Staffing and Supervision, Special Care Provisions and Emergencies and Physical Plant are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 21, 2025. A POC submitted after this date will not appear on the public website.
Violations
3The following was observed within the reach of children in the City Play Place: three light fixtures inside the playhouses with off centered cover plates resulting in gaps between the wall and fixtures; open light socket (no bulb); hole in the inside wall of one of the playhouses; three sprinkler heads with sharp edges and missing caps; two missing sections of the wood trim of a window; and a missing trim piece exposing the sharp tips of six screws.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On January 29, 2025, Classroom #1 was in the City Play Place play area. There were 13 children, ages 2 years old, and three staff present. Staff #2 left City Play Place. Staff # 3 and #4 remained in an area where they did not have sight supervision of the children playing in the City Play Place houses for a period of more than five minutes.

A child busted their lip leaving blood on the floor of the City Play Place. Staff #1 wiped up some of the blood with a tissue but did not clean nor sanitize the surface. After the group left the room, the Licensing Inspector observed additional blood on the floor and in one of the playhouses. The blood on the floor had been stepped in and tracked through the room. The blood was neither cleaned nor sanitized until after the Licensing Inspector intervened.
