Inspector Notes
An unannounced, on-site focused inspection was initiated on April 21, 2025, and completed on April 21, 2025 as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as a result of the last inspection related to Staffing and Supervision, Physical Plant and Special Care Provisions and Emergencies. The on-site inspection began at 3:20 p.m. and ended at 4:13 p.m. The inspector reviewed compliance in the areas listed above. This inspection included a tour of the facility, interviews and observation.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
