8VAC20-780 Special Care Provisions and Emergencies

During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on April 17, 2024 completed on April 21, 2024 in response to a complaint received by the licensing office on April 16, 2024 relating to allegations of Physical Plant and Special Care Provisions and Emergencies. This inspection included document review, tour of the facility, interviews and observation.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May 1, 2025. A POC submitted after this date will not appear on the public website.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
The water fountains on the playground were not working.
Plan of Correction: Facilities has been asked to turn on immediately.
Standard 8VAC20-780-520-C
If diaper cream is used, written parent authorization noting any know adverse reactions shall be obtained.
On April 14, 2025 and April 16, 2025, diaper was used on Child #1 and the center did not have parent authorization.
Plan of Correction: Corrected when mom came at pick up time.
