During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on May 15, 2025 completed on May 27, 2025 in response to a complaint received by the licensing office on May 8, 2025 relating to allegations in the areas of Staff Qualifications and Training, and Staffing and Supervision. This inspection included document review, tour of the facility, interviews and observation.

The preponderance of evidence gathered during the investigation supports one the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on June 5, 2025. A POC submitted after this date will not appear on the public website.

REPEAT VIOLATION
Children under 10 years of age always shall be within actual sight and sound supervision of staff.
Child #1, age 14 months, had been left alone in the bathroom for a nap on at least two occasions, April 21, 2025 and April 22, 2025, and was out of sight and sound supervision while staff cleaned up from lunch and tended to other children.
Plan of Correction: Teachers involved were placed on admin leave until an investigation was completed. It was determined that one of the teachers failed to keep the child in sight and sound was terminated.
