During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on 08/04/2025 completed on 08/20/2025 with local agencies in response to a complaint received by the licensing office on July 31,2025, relating to allegations in the area of Programs. This inspection included interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 2, 2025. A POC submitted after this date will not appear on the public website.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
The following occurred on July 30, 2025: Staff #1 and Staff #2 required thirteen two-year-old children to stay seated at their tables for approximately ten minutes while staff cleaned up after an activity. Staff #1 forced children onto chairs on seven occasions, got within an inch of a child's face to correct behavior on ten occasions, and pulled children by the top of their arms on three occasions. During one of those occasions, Staff #1 held the top of a child's arm for approximately 30 seconds while the child tried to get away and then while holding the top of the child's arm pulled the child around the room and forced the child onto a chair.
Staff #2 put their face within an inch of a child's face in attempt to correct behavior on four occasions and forced a child onto a chair. Children had to stand in line for approximately two minutes before leaving the room, during which one child grabbed another. Staff #2 grabbed both of the child's hands and held them together above the child's head for at least one minute.
Staff #2 pulled a child by the ankle to prevent the child from leaving the group.

Staff were given additional training to complete but have since terminated their employment with Children of America.
Additional corrections:
Management will discuss and model appropriate handling of children and behavior guidance for all ages.
