Areas and equipment of the center shall be maintained in a clean, safe and operable condition.
In the City Play Place room, there was a sprinkler head in the ceiling of a playhouse that was missing the cover which exposed sharp edges within reach of the children.
In the Preschool Two room, there was visible dirt on the wall around the light switch in the bathroom.
In the Two?s room, within reach of the children, there was a piece of the shelf missing exposing the particle board and a wood dowel.

The director put in a work order to have the sprinkler head cover to be replaced in
the room in city place, the room was closed off for children use.
The teachers were given scrubbers and cleaning materials to use to wash the walls in the classroom and bathroom. The teachers were told the we would be cleaning the bathrooms daily during resting time. The management team will make sure each bathroom is cleaned daily,
The shelf in the Twos room was removed and new shelves are being ordered.
Additional corrections
The management team will ensure that the rooms are clean and organized. New shelves are ordered and will be replaced as soon as they come in.
City place will be checked daily by management and teachers to ensure all items are safe.

There were four electrical outlets in the Preschool Two room that did not have a protective cover.

Additional outlet covers were provided to the teachers to keep in the room to ensure that this doesn't happen anymore.
The teachers are required to do a check of the room every day before the room can be opened to ensure the outlets are covered, they are required to do spot checks daily to ensure they are always covered.

Children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks. Children?s hands shall be washed with soap and water after toileting and any contact with blood, feces, or urine.
In the Two?s classroom, eleven of the sixteen children did not have their hands washed after lunch.
In the Infant room, Staff changed an infant?s diaper and the infant?s hands were not washed afterwards.

Additional corrections
The Director will walk around to ensure that hand washing is being implemented when meals ace served and diapers are changed, there will also be a verbal training at the next staff meeting on 12/10/2025 about hand washing.
