During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on January 21, 2026 and completed on February 3, 2026 in response to a complaint received by the licensing office on January 15, 2026 relating to allegations of Staff Qualifications and Training, Staffing and Supervision, Programs, Special Care Provisions and Emergencies and Special Services. The inspector reviewed one children?s records and three staff records on-site. This inspection included document review, tour of the facility, interviews and observation.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 11, 2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
REPEAT VIOLATION
Areas and equipment of the center shall be maintained in a clean, safer and operable condition.
In the Infant C room, a child age pulled an unsecured wooden play kitchen over and onto that child and another which knocked both children to the ground. Staff had to pull the kitchen off of the children.
There were six chairs in the Preschool classroom and six chairs in the Preschool II classroom that were cracked on the backrest under the bolts which created a pinching hazard.
In the School Age Classroom, the wood on one cabinet was broken and the hinge was visible from the outside.

Kitchen set removed from Infant C immediately. Chairs removed from preschool classroom immediately. Cabinet due to be fixed; on task list for maintenance.
Additional corrections
Director will ensure that teachers are using their classroom Maintenance Logs to report any broken items in the classroom, and will be collected by management on a daily basis.
Standard 8VAC20-780-270-C
In inside areas occupied by children, the temperature shall be maintained no lower than 68 degrees Fahrenheit.
Around 9:30 a.m. on January 21, 2026, the temperature in the Preschool classroom was 59 degrees Fahrenheit. One child had a coat on with the hood up, one child had a sweatshirt with a hood on, and the other ten children had long sleeves, sweatshirts and jackets. The staff were both wearing jackets. One child stated that he was cold, and staff told him to put his coat on. Staff stated that the classroom had been cold all morning.
At 9:25 a.m. on February 3, 2026, the temperature in the School Age room was 65 degree Fahrenheit. There were four children present in long sleeves and one staff member wearing a jacket. The center?s thermometer on the wall was showing 62-64 degrees Fahrenheit during the inspection.

HVAC is closely monitored in the morning before open, and throughout the day and ongoing.
Additional corrections
Maintenance has ordered a review of the HVAC units, and during this time, the Director will continue to maintain appropriate temperatures in the school at all times.
Standard 8VAC20-780-350-B-1
The staff-to-children ratio for children Birth to 16 months is one staff to four children.
On January 21, 2026, there were nine children with two for staff for approximately ten minutes. Then two additional children arrived and there were eleven children with two staff for an additional 15 minutes. The youngest child was ten months old.

Staff schedules have been adjusted to have teachers available ahead of the need, so that ratio's will be maintained.

Director will continue reviewing schedule and will ensure teachers are in the
classroom to meet ratio at the appropriate times.
Standard 8VAC20-780-400-B
REPEAT VIOLATION
Behavioral guidance shall be constructive in nature, age and stage appropriate and shall be intended to redirect children to appropriate behavior and resolve conflicts.
On January 21, 2026, Staff #4 was heard shouting at the children in the Preschool II classroom several times.

Teacher separated from employment after issuing a termination notice in regards to raising her voice in the preschool classroom.
Additional corrections
Director will ensure that appropriate tone is used in the classroom ongoing, and will follow our Zero Tolerance policy.
Standard 8VAC20-780-430-I
If combs, toothbrushes or other personal items are used, they shall be individually assigned.
Staff stated that pacifiers are individually assigned by labeling the pacifier with the child?s name.
In the 2?s classroom, there were two pacifiers that were not labeled with a name.
In the toddler room, there was one pacifier that was not labeled with a name.

All pacifiers labeled in the classroom using a permanent non-toxic marker, and will be monitored ongoing for any pacifiers.
Additional corrections
Director will perform daily checks for pacifiers to ensure they are labeled appropriately.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure as it occurs.
On January 21, 2026, there were 15 children present in the Preschool classroom, but the attendance sheet showed arrival of 16 children. One child had left, and the departure was not recorded.
On February 3, 2026, four children were present in the School Age classroom, and the attendance sheet showed only three children had arrived. There were 14 children present in the Preschool classroom, but the attendance sheet showed arrival 15 children. One child had left, and the departure was not recorded.

Rosters fixed during visit, and daily monitoring of roll call sheets will continue ongoing.

Director will perform daily checks of the rosters in the classroom to ensure accuracy of attendance.
