During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site inspection was initiated on March 5, 2026 and completed on March 20, 2026 in response to a complaint received by the licensing office on March 3, 2026 relating to allegations of Administration, Staff Qualifications and Training, and Staffing and Supervision. This inspection included document review and interviews.

The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on April 2, 2026. A POC submitted after this date will not appear on the public website.

Standard 63.2-1509-A
SYSTEMIC VIOLATION
The Code of Virginia requires persons employed in a child day program that have reason to suspect that a child is abused or neglected shall report the matter immediately to the county or city wherein the child resides or wherein the abuse or neglect is believed to have occurred or to the Department's toll-free child abuse and neglect hotline.

In early February 2026 five staff saw marks on Child #1 and suspected an incident of child abuse but did not report to the Department of Social Services.
Plan of Correction: Report created once administration were informed that the report was not sent. All staff completed the CWSE 5692 -Recognizing & Reporting Child Abuse & Neglect training and completed by 03/13/2026.
Additional corrections
Administration will be reporting all suspected abuse and neglect regardless if the staff do not report, to ensure that reports are sent within the required
timeframe of 24-48 hours.
Standard 8VAC20-780-240-C
SYSTEMIC VIOLATION
Orientation shall include recognizing child abuse and neglect and the legal requirement for reporting suspected child abuse as required by ? 63.2-1509 of the Code of Virginia.

The orientation provided to staff did not include the legal requirements for reporting suspected abuse and neglect.
Plan of Correction: All staff completed the CWSE 5692 -Recognizing & Reporting Child Abuse & Neglect training and completed by 03/13/2026.
Additional corrections
Staff will maintain these records on an annual basis, as well as requiring all new staff entering the program to complete CWSE 5692 -Recognizing & Reporting Child Abuse & Neglect before working with children.
Standard 8VAC20-780-340-A
SYSTEMIC VIOLATION
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On February 3, 2026, four staff saw scratches on Child #1?s back and bruises on the child?s leg. Per one staff, the marks looked like four red welts that were wrapped around the child?s leg. Another staff said the marks looked like a handprint. Although abuse was suspected, none of the four staff reported the incident to the Department of Social Services, as required.

On February 4, 2026, a fifth staff saw marks on Child #1, showed the marks to two staff who noted they had seen the same marks the day before and was told it was ?being handled?.

On February 27, 2026, A staff person saw new marks on Child #1 and reported both the new marks and the marks found previously to The Department of Social Services.
Plan of Correction: Administration will be reporting all suspected abuse and neglect regardless if the staff do not report, to ensure that reports are sent within the required timeframe of 24-48 hours.
