Inspector Notes
An unannounced renewal inspection was conducted today from 11:00am-1:00pm. There were 95 children (infants-5yrs.) directly supervised by 22 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, napping, being bottle fed, listening to stories, tracing letters and eating lunch. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized.

5Evidence:
1.Staff #2's (DOH: 2/2/2023) file contains an out of state criminal history check that was completed on 2/6/2023.
2. Staff #5's (DOH: 10/24/2022) file contains an out of state criminal history check that was completed on 11/1/2022.
Evidence:
Staff #6 stated that there were no children with food allergies in their classroom. The written list of allergies, sensitivities and dietary restrictions confirmed that Child #8, that was present in the classroom, is allergic to shellfish.
Evidence:
The medication for child #6 (Benadryl) did not have an authorization form signed by the parent. This medication was administered on 2/22/2023.

The prescription medication for Child #7 was not in the original container with a prescription labeled attached.

Findings of the most recent facility inspection (dated 9/13/2022) were not posted.
