Inspector Notes
An unannounced monitoring inspection was initiated on Monday, May 13, 2024, and concluded on May 16, 2024. On May 13, the inspector was on site from 11:15am to approximately 3:05pm. There were a total of 59 children in care in the direct care of 12 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The center is equipped with toys and supplies and items were available to the children. The required postings were reviewed and found to be in compliance. Transportation is provided and medication is administered when necessary. Five children's records and five staff records were reviewed. Additional documentation was submitted to the inspector on May 16, 2024.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence: The TB screening in the record of Staff #2 (DOE: 04/03/2023) was dated 04/05/2023. During interview, a member of management confirmed the TB screening was completed on 04/05/2023.
Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children.
Evidence: The most recent TB screening in the record of Staff #3 (DOE: 06/01/2020) was not signed by a physician, the physician's designee or an official of the local health department.
During interview, a member of management confirmed the TB screening did not contain the required information.
Evidence: A long-term prescription medication was observed for Child #1, but no written parental authorization was present at the center for this medication.
During interview, a member of management acknowledged the form on file did not include parental authorization for the medication the center had on site for Child #1.
