The vendor did not ensure two staff annually completed the required department sponsored health and safety update training. The record for Staff #4 contained an annual health and safety update certificate from 2023. The updated annual health and safety training was required to be completed seven months ago. The record for Staff #5 contained an annual health and safety update certificate from 2023. The updated annual health and safety training was required to be completed one month ago.
Plan of Correction: The staff members will be required to take the trainings as soon as possible.
