Inspector Notes

A monitoring inspection was initiated and concluded on 10/27/2021. The focus of the inspection was to determine whether the provider had corrected or is in the process of correcting previously cited violations in the areas of standards referenced above. There were 7 children present, ranging in ages from 5 months to 2 years old, with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical environment and equipment and care of children. One caregiver record were reviewed.

Standard 22VAC40-111-50-C
Based on observation, the provider did not keep information contained in a child's record confidential.
Evidence:
1. The provider had a paper on the wall that listed a child's name and their allergy for others to view.
Plan of Correction: I will cover the information regarding children's allergies.
Standard 63.2-1720.1-B-4
Based on interview, the provider failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
1. Assistant #1 was hired on 8/02/2021 and the provider stated the out of state criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry had not been obtained or sent for the State of AZ.
Plan of Correction: I will work on this today and send the results once I receive them.
