Inspector Notes
An unannounced renewal inspection was conducted today from 8:50 am-10:45 am. There were 8 children (21 points) supervised by 2 direct caregivers. The home, 2 staff records, 5 children?s records, evacuation drills, and emergency supplies were inspected. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Violations
4Evidence: Child #1's record did not contain documentation of physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Date of attendance for child #1 is 5/15/2019.
Evidence: A bottle of hand sanitizer was next to three containers of infant formula and infant oatmeal in a white cabinent next to the refrigerator accessible to children.
Evidence: Child #5's record did not contain a signed acknowledgment for the authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. Child #5's date of attendance is 9/12/22.
Evidence: 2 of 2 bottles in the refrigerator were not labeled with the full names and dates of child #4 and child #5.
