Inspector Notes
An unannounced renewal inspection was conducted today from 10:30am-12:30pm. There were 42 children directly supervised by10 staff. The physical plant, 7 staff records, 6 children?s records, 4 renewal applicant records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, being fed, playing outside, and transitioning between activities. Diapering and hand-washing procedures were also observed. Staff and child interactions were observed to be constructive in nature and age appropriate. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Evidence:
A structural wooden platform in the Older Toddler classroom was observed with a chip in the wood approximately the size of a dime. The edges of the hole were sharp to the touch.
However, because it can pose a hazard to the children, a work order was submitted to the Federal
Government. They came in on Friday, Sept. 30 th and fixed this hole.
Evidence:
1. A canister of disinfectant wipes was observed on a low counter in the Preschool classroom.
2. A bottle of spray cleaner was observed in an unlocked cabinet in the Young Toddler classroom.
and left it on her desk area. She was reminded of making sure all cleaning materials need to be in a
locked cabinet immediately.
1) The canister was headed to the recycle unit located in the kitchen area. The teacher was
planning on taking this out during her break. The teacher was reminded to keep these types of
materials higher up and out of the reach of children, until it can be disposed of properly.
2) The unlocked cabinet has been submitted to the Federal government to replace the lock, as it is
broken.

Evidence:
An unlabeled bottle containing a green liquid that a staff member identified as "cleaning solution" was observed in the Young Toddler classroom.

possible violations.
Evidence:
1. In the Older Toddler Classroom an open shelving unit was observed to contain open and accessible jars and containers of: pony beads, small rocks, small balls, glitter, and metallic confetti, which pose a choking hazard for children under the age of three.
2. In the Older Infant room an open container of small "googly eyes" was observed on a low shelf accessible to children, and posed a choking hazard.
3. In the Infant classroom, an unsealed sensory bottle containing dry rice and beans, and a canister containing small plastic pieces, were observed on open shelves accessible to children.
Sensory shelf in the Older Toddler room, the materials on the shelf and the shelving unit have been
removed. Violation #2 and #3 were addressed with staff and these items have been removed from these
rooms. Sensory bottles are to be sealed properly, with glue.
Evidence:
A list containing confidential student medical information was observed posted in publicly observable areas in the Older Toddler and Older Infant classrooms.
teachers have been instructed to put all information regarding medication/health related items on the
inside of the locked cabinet door. This has been classified as a ?null risk assessment.? We are unique
that we are on a federal location and our classrooms are restricted to public access. In fact, parents are
only allowed in the hallways and not in the classrooms.

Liquid allergy medication and an epinephrine injector were observed inside of an unlocked storage box in the Older Toddler Classroom.

and accompany the product when the product is handed to the teacher.
