Inspector Notes
An unannounced renewal inspection was conducted today from 10:30am-12:30pm. There were 42 children directly supervised by10 staff. The physical plant, 7 staff records, 6 children?s records, 4 renewal applicant records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, being fed, playing outside, and transitioning between activities. Diapering and hand-washing procedures were also observed. Staff and child interactions were observed to be constructive in nature and age appropriate. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, renewal applicants did not have the results of an out of state child abuse and neglect search by the end of their 30th day of employment, or the results of a criminal record check prior to the first day of service.

Based on observation, areas of the center were not maintained in a safe condition.

A structural wooden platform in the Older Toddler classroom was observed with a chip in the wood approximately the size of a dime. The edges of the hole were sharp to the touch.

Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A canister of disinfectant wipes was observed on a low counter in the Preschool classroom.
2. A bottle of spray cleaner was observed in an unlocked cabinet in the Young Toddler classroom.

and left it on her desk area. She was reminded of making sure all cleaning materials need to be in a
locked cabinet immediately.

materials higher up and out of the reach of children, until it can be disposed of properly.

Sensory shelf in the Older Toddler room, the materials on the shelf and the shelving unit have been
removed. Violation #2 and #3 were addressed with staff and these items have been removed from these
rooms. Sensory bottles are to be sealed properly, with glue.

teachers have been instructed to put all information regarding medication/health related items on the
inside of the locked cabinet door. This has been classified as a ?null risk assessment.? We are unique
that we are on a federal location and our classrooms are restricted to public access. In fact, parents are
only allowed in the hallways and not in the classrooms.
