Inspector Notes
An unannounced monitoring inspection was conducted today from 10:30am-12:30pm. There were 58 children directly supervised by 10 staff. The physical plant, 7 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, napping, and reading. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

8Evidence:
The criminal record check for Staff 2 was conducted on 3/23/2018.
Evidence:
Staff 6's record does not contain the results of a criminal record check for a state she has lived in within the preceding five years.
Evidence:
Staff 1 and Staff 2's records do not contain a completion certificate for the Virginia Department of Education-sponsored orientation course. Staff 1 was hired in November 2021. Staff 2 was hired November 2022.

Evidence:
1. In the two year old classroom, a wooden toy microwave cabinet was observed to have cracked and splintering wood on the back corner in an areas accessible to children.
2. The wooden planks at the base of the playground ramp were observed to be detaching from the ramp and exposing the finishing nails used to hold the planks in place.
3. Two steak knives were observed in an unlocked cabinet in the Young toddler classroom.
Repeat: Teachers have been instructed on safety practices and are required to take a refresher course on classroom safe environments. The teachers are required to take this training and complete it no later than 4/28/23.
Evidence:
On the date of inspection the director could not produce written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.
Evidence:
A bottle was observed in the refrigerator in the Infant classroom without a label indicating the child's name or the date.
A reminder notice was posted to parents outside of each infant classroom listing the parent?s responsibility on labeling the bottles and any food brought in from home- 4/21/23.
Evidence:
Child 4's record did not contain a written care plan for a diagnosed food allergy, to include instructions from a physician regarding food to which the child is allergic, and steps to be taken in the event of a suspected or confirmed allergic reaction.
