Inspector Notes
A monitoring inspection was initiated on 5/19/2022 and concluded on 5/19/2022. There were a total of 11 children (29 points) observed with 2 caregivers present. The children were engaged in the following: toy play, legos, stories, singing and naps. A total of 2 child records and 2 caregiver records were reviewed. No medications on site. Reviewed fire drills and emergency preparedness plan. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
2Evidence: Child #1, date of first attendance 1/10/2022 does not have a record of the provisions of the family day home's emergency preparedness plan.
The provider said she did not receive the form back from the parent.
Evidence: Diaper ointment was observed above the changing area for child #1, 2,and 3. The diaper ointments were not labeled with the child's name and there was no parental authorization on file.
The provider said she was using the diaper ointments for all three children but did not have authorizations from the parents.
The parents will complete an authorization form.
