Inspector Notes

An unannounced monitoring inspection was conducted today from 10:25am-12pm. There were 8 children (20 points) directly supervised by the provider and 2 assistant caregivers. The home, 3 caregiver records, 5 children?s records, 3 household member?s records, evacuation drills, and emergency supplies were inspected. Children were observed playing freely, having lunch, and preparing for naptime. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain a sworn state or affirmation every five years for 1 of 3 household members .

Evidence:
1. The most current sworn statement for Household Member #3 was dated 09/02/2018
Plan of Correction: A new sworn statement will be completed today.
Standard 8VAC20-800-120-B
Based on record review, 2 of 3 caregiver records reviewed did not contain all required information.

Evidence:
1. The records for Caregiver #2 (start date 2/6/2024) and Caregiver #3 (start date 2/15/2024) did not contain documentation of verification of age.
2. The record for Caregiver #2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.
Plan of Correction: Provider will request the missing information.
Standard 8VAC20-800-90-A
Based on record review, the provider did not obtain documentation a child has been adequately immunized before the child may attend the family day home for 1 of 5 children's records reviewed.

Evidence:
1. Child #4 began attending the family day home on 09/20/2023. The immunizations in the record for Child #4 were dated 09/22/2023.
Plan of Correction: The provider will obtain immunizations before the child's first day.

Based on record review, the family day home did not obtain documentation of additional immunizations every 6 months for one child.

Evidence:
1. Child #2 began attending the family day home on 09/05/2023. The most current documentation of immunizations in the record for Child #2 were dated 08/25/2023.
Plan of Correction: Provider will request updated immunizations after the child completes their yearly physical.
