During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 7/8/2025 and completed on 7/8/2025, as a part of the licensure period. The on-site inspection began at 10:28am and ended at 11:33am. The inspector reviewed compliance in the areas listed above. There were eight children present and two caregivers; the point total was 16. The inspector reviewed five children?s records and six caregiver/household member(s) records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
The provider is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The provider did not request a search of the child abuse and neglect registry from one state for Staff #2, who has been employed for more than 4 months and was working with children at the time of the inspection.
Plan of Correction: Will submit the request for Staff #2's out of state central registry background check.
Standard 8VAC20-800-170-B
The tuberculosis (TB) screening form must have been completed within 30 days of hire and be signed by a physician, physicians' designee, or an official of the local health department.

Staff #2's TB screening form was completed 157 days prior to the hire date.
Plan of Correction: Will have Staff #2 obtain an updated TB screening form.
