During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/06/25 and completed on 10/09/25. The on-site inspection began at 2:45 pm and ended at 4:40 pm. The inspector reviewed compliance in the areas listed above. There were 28 children present and four staff. The inspector reviewed five children?s records on site and three staff records on-site and electronically on 10/8/25, 10/9/25, 10/13/25.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A tuberculosis (TB) screening is required every two years form the last screening.
Staff #1updated screening was due 3 years ago. The updated screening has not yet been completed.
Plan of Correction: TB screen received 10-13-2025.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean and safe condition.
There was a tote under the sink in the art room, full of water. The sink has been leaking at least since the summer. This area is often used by the children.
Plan of Correction: Plumber called. Scheduled to be fixed by 11-7-2025.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.
During the inspection, three children under the age of 10 were unsupervised on three separate occasions. Each child was unsupervised for approximately two minutes.
