Inspector Notes
An inspection was begun on 01/24/2023 in response to a self reported incident at the facility.
Based on multiple staff interviews, the preponderance of evidence supports that the center failed to ensure care, protection and guidance for a child in care when the child was given a known food allergen.

3Evidence:
On January 20, 2023 a staff member responsible for distribution of lunch provided a child a known allergen. The child has a documented food allergy to cow's milk, a placemat indicating the allergy and a milk substitute is provided for the child. Staff stated when the kitchen staff do not send the milk substitute to the classroom with meals/snacks, the child is to drink water. (Staff A)
Evidence:
On January 20, 2023 at approximately 12:05 PM Child #1 was given a known food allergen. The child has a documented food allergy to cow's milk . When lunch arrived in the classroom, there was no milk substitute provided for the child. Staff A poured cow's milk into the drinking cup at the child's seat/ placemat instead of providing water. The child drank some of the milk, and stated " he hated the taste" of the milk, began to gag and then vomited. Symptoms progressed into a full anaphylaxis episode. The child was given emergency medications and transported to a local hospital.
Evidence:
On January 20, 2023 Staff A did not follow the proper procedure for ensuring a child did not come into contact with a known food allergen (cow's milk). The established policy for prevention of exposure is to provide water to Child #1 if a milk substitute (oat milk) was not included with meals/snacks that are delivered to the room. The staff person provided cow's milk to the child resulting in an allergic reaction.
