Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 09/25/24, as a part of the licensure process. The on-site inspection began at 11:00 AM and ended at 2:24 PM. The inspector reviewed compliance in the areas listed above. There were 108 children present and 22 staff. The inspector reviewed 6 children?s records and 6 staff records on-site.
This inspection included; document review, tour of the facility, interviews, observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/4/24. A POC submitted after this date will not appear on the public website.
Violations
2The long term authorization for one medication authorizes a different medication than what is present at the center for Child #1.
Parent was notified on 9/30/24 and given a new form to reflect the medication on hand.
1 of 6 reports reviewed did not include any future action to prevent recurrence.
1 of 6 reports reviewed was dated 8/14/24; however, the parent signed and dated the report 9/11/24.
1 of 6 reports reviewed had the time of injury recorded as the same time the parent was notified.
