An unannounced inspection was initiated on 4/21/25 and completed on 4/29/25, in response to a complaint received by the licensing office on 4/18/25 relating to allegations of staffing and supervision. This inspection included: document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
