Inspector Notes

A monitoring inspection was initiated on November 29, 2022 at approximately 2:45 PM to 4:00 PM. There were 27 children present, ranging in ages from toddlers to preschool, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Based on record review and interview, one staff record did not have documentation of an updated central registry finding every five years from the date of the most recent Central Registry finding.

Evidence:
1. The record for staff #2, had an original Central Registry finding on 03/01/17. The Central Registry was updated on 04/27/22. Central Registry finding should be updated every five years from the date of the most recent Central Registry finding.

Based on observation, the center did not ensure all hazardous substances such as cleaning materials insecticides and pesticides, were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In room #1, one container of Lysol and one container Clorox Fabric Sanitizer, both labeled "Caution" and Keep Out of Reach of Children" were in a unlocked closest.
2. In room #2, one container of Clorox and one container of Glade Air Freshener, both labeled "Caution" and Keep Out of Reach of Children" were above the changing table. It was not diapering time when hazardous substances were observed.
3. In room #4, one container of disinfectant wipes labeled "Keep out of reach" and "Danger" was on top of the children's cubbies.
