Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 09/10/2024 and completed on 09/10/2024. The on-site inspection began at 11:25am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 30 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 09/20/2024. A POC submitted after this date will not appear on the public website.

Violations
3Evidence:
1. Staff #3 (date of hire 09/03/2024) had a tuberculosis screening that was dated on 9/10/2024.
2. Leadership acknowledged that the document was late.
Evidence:
1. In the preschool room, there was a metal baseboard that had fallen and was loose resulting in a crack that contained sharp edges that could hurt or harm a child.
2. Leadership acknowledged the baseboard was broken.
Evidence:
1. During nap time in the 3 year old classroom, there was one cot observed with a bottom cover but not a top cover and another cot observed missing a bottom cover without a top cover.
