An unannounced monitoring inspection was initiated and completed on 1/14/25 in response to a self-report received by the licensing office on 12/27/24 relating to an injury requiring outside medical treatment. This inspection included document review and interviews. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
