An unannounced code compliance inspection was initiated on 11/13/2023 and concluded on 11/13/2023 in response to a complaint received on 10/15/2023 regarding supervision and ratios. The inspector reviewed compliance in the areas of administration, staffing and supervision, and procedures. Interviews and observations were conducted.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on interview and observation, the center failed to ensure the required ratio of one staff to four children for children ages zero to sixteen months.
Evidence:
In the Rainbow room only one staff member was observed with eight children. The assistant director verified the youngest child in the class was 10 months.
