An unannounced, on-site focused inspection was initiated on 03/12/2025 and completed on 03/12/2025, as a part of an Intensive Plan of Correction (IPOC).The Department requested that the provider complete an IPOC as result of the last inspection related to staffing, supervision, and special services.The on-site inspection began at 2:51 pm and ended at 4:30 pm. The inspector reviewed compliance in the areas listed above.
