Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 06/12/2025, as a part of the licensure period. The on-site inspection began at 9:30a.m. and ended at 10:30pma.m . The inspector reviewed compliance in the areas listed above. There were 4 children present and 2 caregivers; the point total was 11. The inspector reviewed 4 children?s records and 2 caregiver and 1 household member record on-site. This inspection included: document review, tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
