Inspector Notes

An unannounced monitoring inspection was conducted with the director beginning at 8:53 and ending at 10:55 AM. There were 31 children, ages ranging between 4 months and 12 years old, in the direct care of 8 staff. The children were observed during circle time, playing with age appropriate toys, and interacting with staff. Five children?s records were reviewed and in compliance. Four staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and battery-operated radio were observed and in compliance. The most recent emergency escape practice drill was documented as 11/22/2022. The most recent practice shelter-in-place drill was documented as 3/08/2022. The most recent health inspection was dated 10/18/2022. The most recent fire inspection was dated as 10/03/2022.

Based on observation, the facility staff did not ensure that all areas and equipment inside the center were maintained in a safe and operable condition. Evidence: The metal edge of the sink is lifting from the counter in the toddler and 2?s classroom. The edge of the sink metal and sharp creating a scratching/cutting hazard.
Plan of Correction: The director placed a request with the maintenance department to have the sinks repaired.
Standard 8VAC20-780-570-E
Based on observation, the facility staff did not ensure that all prepared infant formula shall be dated and labeled with the child's name. Evidence: There were nine bottles in the infant room?s refrigerator that were not dated.
Plan of Correction: The director spoke with the staff regarding checking the bottles once they are received from the parents. If the parents have not labeled and dated the bottles the staff in the infant room will ensure that they are labeled and dated.
