During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced monitoring inspection was initiated and completed on 9/16/25 in response to a self-report received by the licensing office on 9/5/25 relating to behavioral guidance. The inspector reviewed one staff record on-site. This inspection included document review and interviews. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/6/25. A POC submitted after this date will not appear on the public website.

Behavioral guidance is required to be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 9/5/25 at approximately 10:15am, Staff #1 was observed trying to keep a group of toddlers at the table for a painting project. Staff #1 ?forcefully? put Child A back onto their seat two times. Staff #1 then grabbed Child B by one arm to prevent them from leaving the area and the child?s arm went one direction and their body went another direction.
Plan of Correction: The incident was directly observed by both the Center Director and Assistant Director, and immediate action was taken. The teacher involved was promptly removed from the premises and terminated. In alignment with our commitment to transparency and child safety, the Center self-reported the incident to the State licensing agency.
