Inspector Notes
An unannounced complaint inspection was conducted on 9/19/2022 to investigate an allegation received by the department related to special care provisions and emergencies - over the counter skin products. Based on evidence gathered through interviews and documentation review, the complaint was determined to be valid. A violation notice was issued to the facility. Based on interviews and a review of documentation, it was determined that not all diaper ointment/cream use followed all requirements. Evidence: In the infant class, there were three diaper ointment/creams that were not labeled with the child's name and in the toddler class, there was one diaper ointment/cream that was not labeled with the child's name. The following children, with diaper ointment/cream on site, did not have written parent authorization to apply diaper ointment/cream noting any known adverse reactions: child # 1, #2, #3, #4, #5, #6, #7, #8, #9, #10, #11, #12, #13, #14, #15, #16 and #17.
Plan of Correction: All diaper ointment permission forms have been obtained and/or updated with required information.
