During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 6/16/25 and completed on 6/20/25 in response to a complaint received by the licensing office on 6/13/25 relating to allegations regarding a forbidden action, rough handling; behavior guidance; and soiled clothing. This inspection included document review and interviews. The preponderance of evidence gathered during the investigation does not support the allegations relating to a forbidden action, rough handling and behavior guidance, therefore, that part of the complaint is determined to be not valid. The evidence gathered during the investigation supports the allegation relating to soiled clothing; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/30/25. A POC submitted after this date will not appear on the public website.

When a child?s clothing or diaper becomes wet or soiled, the center is required to ensure that the child is cleaned and changed immediately.

On 6/12/25, Child A, three-years-old, participated in water play at the center from approximately 10:00am-10:40am. Child A was not changed out of their wet clothes following water play and was found to be napping in their wet clothes during pickup at approximately 1:40pm.
