During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/8/25. The on-site inspection began at 11:55am and ended at 1:35pm. The inspector reviewed compliance in the areas listed above. There were 60 children present and 11 staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/17/25. A POC submitted after this date will not appear on the public website.

The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

At approximately 12:05pm on 10/8/25 there were 19 children observed in the ?Prep? classroom for children three- to four-years-old. The written attendance record contained documentation of 11 children present due to children being signed out for an activity but not signed back in. Staff then updated the record and documented 20 children present, while 19 were still present.
Plan of Correction: All staff were retrained on attendance procedures on 10/13/25.
