Inspector Notes

An unannounced monitoring inspection was started in the center on February 5, 2024 from 5:15 PM until 6:10 PM and concluded on February 8, 2024. At the time of entrance 13 children were in care with 2 staff members and 1 volunteer. The sample size consisted of 5 children?s records and 2 staff records. Children were observed playing board games, coloring , racing match box cars and working on homework.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Based on record review and staff interviews, the licensee did not ensure to obtain documentation that each child has received a physical examination by or under the direction of a physician before the child's attendance; or within 30 days after the first day of attendance.

Evidence: The record for Child #5, (first date of attendance in September 2020) did not include documentation of a required physical.

Standard 8VAC20-780-270-A
Based on observation and staff interviews, the licensee did not ensure that in areas used by children of preschool age or younger, fans, when used, shall be out of reach of children and cords shall be secured so as not to create a tripping hazard.

Evidence:
A portion, (approximately 2 ? feet long by 9 ? inches wide) of the top layer of the floor tile, in the center of restroom has been removed and the tile is broken, chipped, has sharp edges and it creates a tripping hazard. This restroom is closest to the backdoor of the center.

Standard 8VAC20-780-80-A
Based on documentation review and staff interviews, the licensee did not ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1.There was not an attendance sheet for the 7 children observed in the childcare multipurpose room in the childcare building/center.
2.There was not an attendance list for the 6 children observed in the room for the Blue Group located in the fitness building/center.
