Inspector Notes
An unannounced monitoring inspection was conducted on 09/09/2022 between the hours of approximately 2:55 p.m. and 4:25 p.m. There were 8 children present with the Provider and 2 assistants for a total of 22 points within the supervision guidelines. The children were observed listening to a story and engaging in free play. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
7Evidence: Caregiver #1 stated that there was not a carbon monoxide detector in the home.
Evidence:
1. The most recent Report of Tuberculosis Screening form for Household Member #1 was dated 11/11/2019.
Evidence:
1. There was chipped paint on the wall by the baby gate between the main daycare room and the area with the high chairs.
2. There was chipped paint on the wall by the cubby shelves in the main daycare room.
3. There was chipped paint on the baseboards by the television in the main daycare room.
4. There was chipped paint on the column in the middle of the main daycare room.
Evidence: In the kitchen, a toaster, toaster oven and coffee pot were plugged in while not in use.
Evidence: The last day that the family day home recorded attendance was 09/07/2022.
Evidence:
1. The diaper changing pad had a cloth cover.
Evidence: The provider did not have a portable battery-operated weather band radio in the home.
