During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 03/24/2026 and completed on 03/24/2026. The on-site inspection began at 2:22pm and ended at 3:47pm. The inspector reviewed compliance in the areas listed above. There were 10 children present and two caregivers; the point total was 19. The inspector reviewed five children?s records and two caregiver records and one household member record on-site. This inspection included document review, tour of the facility, interviews, and observations.

A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat Violation:
Caregivers who work directly with children shall annually attend at least 16 hours of training, to include the department's health and safety update course.

The department's annual health and safety course was due five months ago for staff #2.
Plan of Correction: Assistant is currently completing. I will set a reminder to ensure that it is met.

Lockdown procedures shall be practiced at least annually.

There was not a lockdown drill documented for 2025.
Plan of Correction: I will be sure to document when lockdown drills are done.

The provider shall ensure that a caregiver does not exceed 16 points.

At the start of the inspection on 03/24/2026, Staff #2 was the only caregiver present with 10 children at 19 points, for approximately 19 minutes before staff #1 arrived.
Plan of Correction: I will be hiring an assistant for when I have to run out.

Shelter-in-place procedures shall be practiced a minimum of twice per year.

There was only one shelter-in-place drill documented for 2025.
