Inspector Notes

A monitoring inspection was concluded on 6/3/22. There were 15 children present, with 3 staff supervising. The children were observed enjoying birthday celebrations, and playing in small group activities including but not limited to blocks and home living. A walk through of the facility was conducted. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, emergencies and nutrition. A total of 2 children's records and 2 staff records were reviewed. There were no medications on site.

Standard 8VAC20-780-270-A
Based on observations, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.
Evidence:
1. A home living plastic kitchen set was found on the porch that is used by the children. The plastic oven window was broken, posing an injury hazard.
2. A large metal airplane with green seats had a rusty and broken metal rod that had detached from the rest of the plane. This plane was sitting on the playground, posing an injury hazard to the children.
3. Chipped and peeling paint was found in Staff # 1's Pre-K classroom on purple wall across from the entrance.
4. The playground had an excessive growth of weeds on the ground, Additionally there are branches from a berry tree that are hanging over and within reach of children.
Plan of Correction: 1. Remove broken kitchen set. 2. Remove airplane from playground. 3. Chipped paint repainted. 4. Remove weeds and cut berry branch.

Based on observation, hazardous materials were not kept in a locked place using safe locking method that prevents access by children.
Evidence: A large bottle of hand sanitizer was stored on a shelf by the entrance, in Staff # 2's classroom. This bottle was accessible to the children.
Plan of Correction: The bottle of hand sanitizer was removed during the inspection.
