Inspector Notes

A monitoring inspection was conducted today from 1:00 pm to 2:25 pm. There were 2 children in care of 2 staff members. The children were observed eating snack and engaged in free play. There were no children with allergies or medications on site. 3 staff records, 3 children's records, attendance, emergency drill log, emergency preparedness plan and other center documents were reviewed. Based on records review, the center did not obtain repeat background checks for each employee every five years.
Evidence:
1. Staff # 1's fingerprint based criminal record check (CRC) was last completed on 6/5/18. A repeat CRC was not obtained in five years.
2. Staff # 2's fingerprint based criminal record check (CRC) was last completed on 10/11/18. A repeat CRC was not obtained in five years.
3.Staff # 3's fingerprint based criminal record check (CRC) was last completed on 8/14/18. A repeat CRC was not obtained in five years.
4. Staff # 3's most recent central registry search (CRS) was last completed on 12/11/17. A repeat CRS was not obtained in five years.
Plan of Correction: All background checks will be updated and a follow up will be conducted with the licensing inspector.

Based on records review, the center did not obtain subsequent tuberculosis (TB) screening for employees, every two years.
Evidence:
1. Staff # 1's most recent TB screening was completed on 10/27/20.
2. Staff # 2's most recent TB screening was completed on 5/2/18.
3. TB screenings must be repeated every two years.
Plan of Correction: Updated TB screenings will be obtained.
