Inspector Notes

An unannounced monitoring inspection was conducted from 11:45am - 2:15 pm with the director. There were 97 children, ranging from four months to five years of age, present with 13 staff supervising. Infant care was observed and older children were eating lunch, preparing for nap and napping. Lunch served today: chicken nuggets, tater tots, mandarin oranges and milk. Eight child records and five staff records were reviewed, along with the emergency drill log, written daily attendance, written allergy list, two medications and their authorization forms and allergy care plans. (Repeat Violation) Based on review of five staff records, it was determined that not all staff had documentation of a negative tuberculosis (TB) test/screening completed at least every two years from the date of the initial test or screening. Evidence: the record for staff #2 had a negative TB test/screening dated: 1/10/2022.
